Dear Painsmith Patients

Re: Important Notice: Closure of The Painsmith, PLLC and Transfer of Your Medical Records

On August 3rd, 2026, I sent out notices that I would unfortunately be unable to continue practicing at Painsmith to the same high standard that my patients deserve. I also indicated that we were talking with other local pain management physicians regarding who would be best able to serve my patients moving forward. I have been able to locate another local practice that I feel meets or exceeds the level of care and has the capacity to take on additional patients. My top priority remains making sure your care continues without interruption.

To that end, I have arranged for ReleviiMed, led by Dr. Manuel Ybarra, M.D. and a team of providers, to serve as the custodian of medical records for our patients going forward, and I strongly recommend you reach out to ReleviiMed to continue your care. Their physicians have reviewed our practice’s treatment approach and are prepared to take on our patients, including those with ongoing pain management plans, without a gap in care.
